To Make A Referral . . .

The Anxiety Disorders Clinic is going through a time of restructuring which will result in some changes to our intake procedures.  We have revised the Referral form which also acts as a referral letter so duplication of information is no longer required. 

To be referred to the clinic for consultation or treatment, a physician referral is required.  

A physician referral is not required for research study participation.   Patients may contact our research staff directly (christine@macanxiety.com) or call 905-521-2100 extension 76632 to inquire further.  See our web page outlining the various ongoing research studies.

For Doctor's Offices:  Download the Referral Form and return it to us with the requested information, then fax it to Fax # 905/521-2628 or mail it to the address at the bottom of this page.

Anxiety Disorders Clinic Referral Form

Once the completed referral form is received, the information will be reviewed and you will be notified of the disposition of the case.  The patient's name will then be placed on a waiting list.  You and the patient will be notified directly with the appointment date.

Intake Criteria:

bulletReferrals accepted for consultation, treatment, or for participation in research studies
bulletBetween the ages of 7 to 65 years of age (children under age 7 will be considered on a case-by-case basis)
bulletPatients must have a primary DSM-IV diagnosis of an anxiety disorder or related disorder
bulletPatients with depression are accepted, however, the primary problem must be an anxiety disorder
bulletPatients will not be accepted if they are actively suicidal
bulletIndividuals with a primary diagnosis of psychosis, severe substance abuse, pervasive developmental disorders and personality disorders (i.e., borderline, antisocial) are not accepted into the clinic
